The most critical component is the medical report, which must be filled out by a licensed healthcare professional, such as a family doctor, psychiatrist, or nurse practitioner. This report requires the medical professional to describe the applicant’s condition in depth, detailing how the disability impedes their ability to work, care for themselves, or participate in community life. It is not enough to simply list a diagnosis; the medical professional must provide clinical evidence of functional limitations. Therefore, it is crucial for applicants to choose a doctor who knows their medical history well and is willing to advocate for the severity of their condition.
