Imperfections in winding alignment or magnetic asymmetry generate forces along the vertical axis of the transformer. These forces attempt to compress the windings or, more dangerously, force the inner and outer windings apart (telescoping). This can lead to the collapse of the winding spacers and internal short circuits between layers.
